Mindful Transitions:  Adult Partial Hospitalization Program at Lindner Center of HOPE

PHP helps keep patients out of the hospital, while providing intensive treatment in a safe setting.

Lindner Center of HOPE is committed to providing the full spectrum of care for adults suffering with mental illness and therefore offers an Adult Partial Hospitalization Program called Mindful Transitions.

When an adult’s daily functioning is impaired by mental illness, yet criteria is not met for hospitalization, Mindful Transitions, the Adult Partial Hospitalization Program (PHP), provides intensive treatment in a safe and therapeutic environment, without full hospitalization. Partial hospitalization helps patients progress to the point where standard outpatient appointments can be effective. This program is also used as a step-down program from hospitalization, with the intent of gradually easing an adult back into their home environment.

This group based program operates Monday through Friday from 9 a.m. to 3 p.m. and enrollees attend 5 days per week for an average of 2 weeks. The curriculum offers some flexibility in design, so that the PHP treatment team and the enrollee can work together to create the right treatment plan for each individual.

The PHP program works in conjunction with the enrollees’ established outpatient mental health providers.

Mindful Transitions (PHP) includes:

  • Group therapy (DBT informed)
  • Pharmacotherapy consultation
  • Interdisciplinary assessment
  • Lunch in Lindner Center of HOPE’s dining room
  • In-program medication management by a psychiatrist, as needed
  • Mid-program and discharge communications to outpatient providers

When should you consider partial hospitalization:

  • If you are transitioning from an acute inpatient stay.
  • If your outpatient appointments are not intensive, in-depth or frequent enough to keep you effectively functioning.
  • If you are unable to function socially, professionally or both.
  • If you need more intensive intervention and skills development.

Who will benefit from Adult Partial Hospitalization?

The Adult Partial Hospitalization Program (PHP) at Lindner Center of HOPE will benefit individuals, ages 18 and older, who are experiencing emotional or behavioral issues significant enough to affect daily functioning or quality of life. Patients may be suffering with diagnoses including, but not limited to:

  • Mood disorders, such as depression and bipolar disorder
  • Anxiety disorders

  • Adjustment disorders
  • Thought disorders not requiring hospitalization, such as paranoia and hallucinations

Mindful Transitions PHP Facts

  • Mindful Transitions PHP operates Monday through Friday: 9 a.m. – 3 p.m. (7:30 a.m. to 6 p.m. for patients with eating disorders.)
  • PHP Enrollees check in every morning between 8:30 – 9 a.m. and check out between 3 – 4 p.m.
  • During check in, badges will be issued to enrollee and must be turned in when leaving the facility
  • This is a full-day program where the patient attends five days per week. The treatment team will work with patients to design the right program for each situation.
  • Average duration of the program is 2 weeks. The treatment team will make a recommendation regarding length of attendance based on the individual needs of each patient.
  • Diagnosis of patients must fall within the diagnoses classified in the current edition of the DSM-IV, including but not limited to: affective disorders (depression and bipolar disorder), anxiety disorders, eating disorders, adjustment disorders, thought disorders not requiring hospitalization, such as paranoia and hallucinations
  • Program primarily takes place in group room space on the second floor of Lindner Center of HOPE.
  • Program participants must have an established outpatient provider who can provide mental health services after discharge.
  • Admission process includes: (If patient is coming from within the Lindner Center of HOPE system, services will be transferred to PHP.)
    • Patients are asked to arrive between 7 and 7:30 a.m. on day one for admission.
    • A nursing assessment that will take about one hour on day one.
    • Social work assessment.
    • Physician assessment.
    • Treatment plan recommendations.
  • The program includes:
    • Group therapy
    • Pharmacotherapy consultation
    • Interdisciplinary assessment
    • Lunch in Lindner Center of HOPE’s dining room (Special dietary needs can be accommodated.)
    • In-program medication management by a psychiatrist, as needed
    • Mid-program and discharge communications to outpatient providers
  • Additional program information:
    • The program is group based.
    • Programming starts promptly at 9:00a.m.
    • Groups are primarily skill development, with a DBT informed basis.
    • For patients with an eating disorder, program includes: Meal support, one offsite therapeutic experience per week, nutritional support and meal planning, medication management, labs, individual and family therapy support when needed and specialized group.
