Confidentiality Statement

In accordance with Ohio state law, information regarding a patient/client is strictly confidential and no part of the patient’s medical record may be revealed over the telephone or in person without the patient’s/client’s previous knowledge and written consent.

Information concerning previous admission(s) is also strictly confidential and cannot be revealed to anyone without the patient’s client’s previous knowledge and written consent.

Program staff shall not convey to a person outside of the program that a patient/client attends or receives services from the program or disclose any information identifying a patient/client as an alcohol or drug services patient/client unless the patient/client consents in writing for the release of information, the disclosure is allowed by a court order, or the disclosure is made top a qualified personnel for a medical emergency, research, audit or program evaluation purposes.
